File a Complaint

If you have any questions about filing a complaint please contact a College Patient Advocate. A Patient Advocate can be reached at: 780-423-4764 or 1-800-661-4689.   

To file a formal complaint you must complete both of the following forms

  1. Complaint Reporting Form 
  2. Release of Information Form  

When the College receives your complaint, our Complaints Director will review it and determine how to proceed based on options outlined in the Health Professions Act.

NOTE - We are unable to accept verbal, faxed or emailed complaints without completion of the above two forms.
