• Annual Practice Permit/Professional Corporation Renewal

  Alberta medical practice permits and Professional Corporations must be renewed each year by December 1. The annual Registration Information Form (RIF)/Professional Corporation Form E Statement of Particulars and payment of the annual fee are completed online.

• Certificate of Professional Conduct Form

  • Certificate of Professional Conduct - complete this form online, print it and sign as appropriate. Return to CPSA by fax (780-426-0805) or mail (2700-10020 100 Street NW, Edmonton AB  T5J ON3, CANADA).
  • Please note that processing a Certificate of Professional Conduct can take up to 10 business days. 
  • There is no fee to obtain a Certificate of Professional Conduct if you are a licensed resident on the College's Education or Courtesy register.

• Notification of Change form (for practice & address updates)

  You must notify the College when you: 

  Move outside of Alberta Move within Alberta Change your e-mail address	Return to practice in Alberta Retire from practice

  If any of above apply to you, you must complete a Notification of Change Form. For more information, see the Frequently Asked Questions section or email physicianinquiries@cpsa.ab.ca or call 780-969-4925.

• Pre-Authorized Payment (PAP) Form

  Enrol in a College Pre-Authorized Payment (PAP) Plan to have practice permit and/or Professional Corporation fees withdrawn automatically from your account. There are no administrative fees and the plans can prevent late payment fines.
